- Notify affected individuals following certain breaches when required.
Permitted uses and disclosures
Where applicable, information may be used or disclosed for treatment, payment, healthcare operations, public health, oversight, legal proceedings, law enforcement, safety, and other purposes permitted or required by law.
Uses requiring authorization
Written authorization will be obtained when required, including for certain marketing, sales of protected health information, or uses not otherwise permitted.

- Inspect and obtain copies of certain information.
- Request amendments.
- Request an accounting of certain disclosures.
- Request restrictions or confidential communications.
- Receive a paper copy of the notice.

You may contact FullyKnow's Privacy Officer or the U.S. Department of Health and Human Services Office for Civil Rights. FullyKnow will not retaliate against you for filing a complaint.
